Thank you for your interest in participating in MAP’S workplace giving program.  If your application is successful, you will join hundreds of charities that are eligible to get designated financial gifts from State, Federal, City, County and private employees in the Fall 2008 MAP campaign.

Please review the applications VERY CLOSELY:

2008 State Application  (attach IRS determination letter unless otherwise indicated). When you are done filling out your application, sign it, then mail it back to us. It is an editable PDF, so you can type in the appropriate fields, print it, sign it and mail it back! Send an emailed copy as an attachment, then we can cut and paste your information appropriately, instead of retyping. Easier and more accurate for all of us!
